Objective: To evaluate the efficacy and safety of Nocardia rubra cell wall skeleton (Nr-CWS) in the treatment of persistent cervical high-risk human papillomavirus (HR-HPV) infection. Methods: A randomized, double blind, multi-center trial was conducted. A total of 688 patients with clinically and pathologically confirmed HR-HPV infection of the cervix diagnosed in 13 hispital nationwide were recruited and divided into: (1) patients with simple HR-HPV infection lasting for 12 months or more; (2) patients with cervical intraepithelial neoplasia (CIN) Ⅰ and HR-HPV infection lasting for 12 months or more; (3) patients with the same HR-HPV subtype with no CINⅡ and more lesions after treatment with CINⅡ or CIN Ⅲ (CINⅡ/CIN Ⅲ). All participants were randomly divided into the test group and the control group at a ratio of 2∶1. The test group was locally treated with Nr-CWS freeze-dried powder and the control group was treated with freeze-dried powder without Nr-CWS. The efficacy and negative conversion rate of various subtypes of HR-HPV were evaluated at 1, 4, 8, and 12 months after treatment. The safety indicators of initial diagnosis and treatment were observed. Results: (1) This study included 555 patients with HR-HPV infection in the cervix (included 368 in the test group and 187 in the control group), with an age of (44.1±10.0) years. The baseline characteristics of the two groups of subjects, including age, proportion of Han people, weight, composition of HR-HPV subtypes, and proportion of each subgroup, were compared with no statistically significant differences (all P>0.05). (2) After 12 months of treatment, the effective rates of the test group and the control group were 91.0% (335/368) and 44.9% (84/187), respectively. The difference between the two groups was statistically significant (χ2=142.520, P<0.001). After 12 months of treatment, the negative conversion rates of HPV 16, 18, 52, and 58 infection in the test group were 79.2% (84/106), 73.3% (22/30), 83.1% (54/65), and 77.4% (48/62), respectively. The control group were 21.6% (11/51), 1/9, 35.1% (13/37), and 20.0% (8/40), respectively. The differences between the two groups were statistically significant (all P<0.001). (3) There were no statistically significant differences in vital signs (body weight, body temperature, respiration, pulse rate, systolic blood pressure, diastolic blood pressure, etc.) and laboratory routine indicators (blood cell analysis, urine routine examination) between the test group and the control group before treatment and at 1, 4, 8, and 12 months after treatment (all P>0.05); there was no statistically significant difference in the incidence of adverse reactions related to the investigational drug between the two groups of subjects [8.7% (32/368) vs 8.0% (15/187), respectively; χ2=0.073, P=0.787]. Conclusion: External use of Nr-CWS has good efficacy and safety in the treatment of high-risk HPV persistent infection in the cervix.

La persistencia de lesiones perirradiculares luego del tra- tamiento endodóntico es un problema que requiere del clínico un conocimiento cabal de la histofisiología y de la histopato- logía del sistema de conductos radiculares del tejido pulpar y de los tejidos perirradiculares (periodonto y hueso); además de considerar siempre la posible existencia de enfermedades sistémicas que también pueden actuar como factores de in- fluencia. La presencia de bacterias remanentes a posteriori del tratamiento es considerada como una de las causas principales y más frecuentes para la perpetuación de las lesiones perirra- diculares. Sin embargo, existen otros factores causales, como la existencia de conductos laterales o accesorios infectados y no tratados, la reabsorción dentinaria interna, intercomunica- ciones, cul-de-sacs o istmos; que representan áreas de difícil acceso durante la instrumentación e irrigación. Cuando la cau- sa original se localiza en la zona perirradicular, como en los casos de actinomicosis, reacciones a cuerpo extraño, cristales de colesterol (CRCo) y granulomas o quistes con alto conte- nido de CRCo, la indicación más adecuada es el retratamiento y la cirugía periapical como complemento (AU)


The persistence of periradicular lesions after endodontic treatment is a problem that requires the doctor to have a thor- ough knowledge of the histophysiology and histopathology of the root canal system, the pulp tissue and periradicular tis- sues (periodontium and bone); as well as always considering the possible existence of systemic alterations that can also be influencing factors. Persisting bacteria within the root canal system after treatment is one of the major and most frequent causes for the perpetuation of periradicular lesions. Howev- er, there are other possible causal factors such as the exist- ence of untreated lateral or accessory canals, internal dentin resorption, intercommunications, cul-de-sacs or isthmuses; areas that represent a difficulty in access during instrumen- tation and irrigation. If the original cause is located in the periradicular area, in cases like actinomycosis, foreign-body reactions, cholesterol crystals (CRCo) and granulomas or cysts with high content of CRCo, retreatment coupled with periapical surgery is the best approach to treatment (AU)

Bacterial vaginosis is the most common cause of vaginal discharge and occurs when there is an imbalance in the vaginal microbiota, predominantly composed of Lactobacillus spp. Human Papillomavirus is the most common sexually transmitted virus in the world. Persistent infection with high-risk Human Papillomavirus genotypes is the main cause of the development of cervical intraepithelial neoplasia and cervical cancer. Objective: To investigate the association between bacterial vaginosis and cervical Human Papillomavirus infection and between bacterial vaginosis and cervical cytological abnormalities in adult women. Methods: Cross-sectional study carried out in a gynecology outpatient clinic of the public health network. A total of 202 women were included in the study and underwent gynecological examination with cervical specimen collection. Cervical cytopathological examinations and bacterioscopy by the Nugent method were performed to identify bacterial vaginosis, and PCR and reverse hybridization were carried out for Human Papillomavirus detection and genotyping. Bivariate analysis was performed to investigate the association between bacterial vaginosis and cervical Human Papillomavirus infection, and between bacterial vaginosis and cervical cytological abnormalities. The odds ratio was calculated, with the respective 95% confidence intervals (95%CI) and 5% significance level (p≤0.05). Results: The prevalence of bacterial vaginosis was 33.2% (67/202), the prevalence of cervical Human Papillomavirus infection was 38.6% (78/202) and the prevalence of cervical cytological abnormalities was 6.0% (12/202). Bivariate analysis showed no significant association between bacterial vaginosis and cervical Human Papillomavirus infection (OR 0.69; 95% CI 0.37­ 1.27; p=0.23), or between bacterial vaginosis and cervical cytological abnormalities (OR 0.65; 95%CI 0.17­2.50; p=0.54). Conclusion: In this study, bacterial vaginosis did not represent a risk factor for cervical Human Papillomavirus infection or for the presence of cervical cytological abnormalities in the investigated adult women.


A vaginose bacteriana é a causa mais comum de corrimento vaginal e ocorre quando há um desequilíbrio da microbiota vaginal, composta predominantemente de Lactobacillus spp. O papilomavírus humano é o vírus sexualmente transmissível mais comum no mundo. A infecção persistente com genótipos do papilomavírus humano de alto risco é a principal causa do desenvolvimento de neoplasias intraepiteliais cervicais e câncer de colo do útero. Objetivo: Investigar a associação entre vaginose bacteriana e infecção cervical pelo papilomavírus humano e entre vaginose bacteriana e anormalidades citológicas cervicais em mulheres adultas. Métodos: Estudo de corte transversal realizado em um ambulatório de ginecologia da rede pública de saúde. O total de 202 mulheres foi incluído no estudo e submetido ao exame ginecológico com coleta de espécime cervical. Foram realizados os exames citopatológicos cervicais, a bacterioscopia pelo método de Nugent para a identificação da vaginose bacteriana e reação em cadeia da polimerase e hibridização reversa para a detecção e genotipagem do papilomavírus humano. Análise bivariada foi realizada para investigar a associação entre vaginose bacteriana e infecção cervical pelo papilomavírus humano e entre vaginose bacteriana e anormalidades citológicas cervicais. Foi calculado o odds ratio, com os respectivos intervalos de confiança de 95% (IC95%) e nível de significância de 5% (p≤0,05). Resultados: A prevalência da vaginose bacteriana foi de 33,2% (67/202), a da infecção cervical pelo papilomavírus humano foi de 38,6% (78/202) e a de anormalidades citológicas cervicais foi de 6,0% (12/202). A análise bivariada não demonstrou associação significativa entre vaginose bacteriana e infecção cervical pelo papilomavírus humano (OR 0,69; IC95% 0,37­1,27; p=0,23), nem entre vaginose bacteriana e anormalidades citológicas cervicais (OR 0,65; IC95% 0,17­2,50; p=0,54). RESUMEN

Root canal therapy is the common treatment for endodontic infections. Successful root canal therapy depends on favorable root canal preparation, root canal medication and three-dimensional obturation of the root canal system. The key to successful root canal therapy is to prevent re-infection of the highly complex root canal systems by removing infecious biofilms and bacterial toxins in the root canal system. The present paper reviews the pathogenic mechanism of the Enterococcus faecalis in the harsh environment of root canal system, the inflammation and immunity of refractory periapical periodontitis and the progress of infection control methods.

La COVID 19 es una enfermedad que, habitualmente, no resulta grave en la edad pediátrica, excepto en niños con comorbilidades significativas subyacentes. Es muy importante reconocer los cuadros post COVID, como el síndrome inflamatorio multisistémico (SIM-C) y la COVID-19 prolongada o long COVID que pueden afectar de manera significativa a la población de niños y adolescentes. La pandemia COVID-19 también ha tenido un fuerte impacto en los aspectos sociales, emocionales y nutricionales. El aislamiento prolongado impactó en los controles de salud de niños y adolescentes con enfermedades crónicas y las coberturas de las vacunas de calendario disminuyeron significativamente. Es claro que la vacunación contra el SARSCov-2 en niños, niñas y adolescentes no sólo busca cuidar de su salud; también busca preservar la vida social y presencialidad escolar, reducir el riesgo de los cuadros post COVID-19 y mejorar la inmunidad de rebaño de la población general.


COVID-19 is a disease that is not usually serious in children, except in children with significant underlying comorbidities. Is very important to recognize post-COVID conditions such as multisystem inflammatory syndrome (SIM-C) and prolonged COVID ­ 19 or long COVID, which can significantly affect the population of children and adolescents. The COVID -19 pandemic has also had a strong impact on social, emotional, and nutritional aspects. Prolonged isolation had an impact on health checkups for children and adolescents with chronic diseases, and coverage of scheduled vaccinations decreased significantly. It is clear that vaccination against SARSCov-2 in children and adolescents not only seeks to take care of their health , it also seeks to preserve social life and school presence, reduce the risk of post-COVID-19 conditions and improve herd immunity in the general population.

RESUMEN Dentro de los agentes patógenos en los procesos otíticos bacterianos, se destacan microorganismos como Staphylococcus pseudintermedius, Pseudomona auriginosa, Proteus mirabi-lis, Escherichia coli, Corynebacterium spp., Enterococcus spp. y Streptococcus spp., para los cuales se ha descrito resistencia frente a los antibióticos empleados para combatirlos. En Colombia son pocos los reportes acerca de la resistencia antibiótica de microorganismos causantes de otitis. Por ello, el objetivo de esta investigación fue determinar los agentes bacterianos más frecuentemente aislados en infecciones otíticas de caninos remitidas a un laboratorio veterinario de Medellín durante el 2019 y su resistencia a antibióticos. Para llevarlo a cabo, se realizó un estudio descriptivo transversal retrospectivo. Se analizaron los resultados de los antibiogramas realizados a partir de cultivos bacterianos en muestras óticas remitidas a un laboratorio de referencia de la ciudad de Medellín. Además, se efectuó un análisis de frecuencias para la muestra total. Se encontró que los principales microorganismos bacterianos aislados fueron Staphylococcus pseudintermedius, Pseudomona auriginosa, Proteus mirabili y Staphylococcus aureus. La gentamicina fue el medicamento que mayor porcentaje de resistencia presentó y la cefalexina el que menos resistencia presentó. Se pudo concluir que el Staphylococcus pseudintermedius está presente en más del 60% de los casos de otitis bacteriana. Adicionalmente, se observó una variación de la resistencia presentada por los microorganismos en el tiempo. Estos presentaron mayor resistencia ante los antibióticos aminoglucósidos.


ABSTRACT Among the pathogens in bacterial otic processes, microorganisms such as Staphylococcus pseudintermedius, Pseudomona auriginosa, Proteus mirabilis, Escherichia coli, Corynebac-terium spp., Enterococcus spp., and Streptococcus spp. stand out, for which resistance to antibiotics has been described employed to combat them. In Colombia there are few reports about the antibiotic resistance of microorganisms that cause otitis. For that reason, the purpose of this study was to determine the bacterial agents most frequently isolated from canine ear infections and their resistance to antibiotics from samples of ear secretions sent to a veterinary laboratory in Medellín during 2019. In order to do that, an cross-sectional, retrospective descriptive study was done. The results of the antibiograms performed from bacterial cultures obtained from ear samples sent to a reference laboratory in the city of Medellín were analyzed. A frequency analysis was carried out for the total sample. It was found that the main isolated bacterial microorganisms were Staphylococcus pseudintermedius, Pseudomona auriginosa, Proteus mirabili and Staphylococcus aureus. Gentamicin was the drug with the highest percentage of resistance and cephalexin the one with the least resistance. It was possible to conclude that Staphylococcus pseudintermedius is linked in more than 60% of cases of bacterial otitis and the resistance presented by microorganisms varies over time. The group of aminoglycosides antibiotics was the one which microorganisms are manifesting more percentage of resistance.

Chagas disease (CD) is a severe health problem in countries of Latin America, including Brazil, and is characterised as an acute and chronic infection. CD can affect several organs and cellular tissues, in particular the heart, which can lead to sudden death. This work aimed to review the literature regarding cardiac autonomic function assessed by Heart Rate Variability (HRV) in CD in adult individuals. The search was carried out using the electronic databases PubMed, SciELO, and Web of Science between January 2013 and October 2020. A total of 19 articles were found, of which 14 were included in this review. HRV was demonstrated at baseline conditions, physical exercise, and cardiac rehabilitation. Quantifying CD progression assessed by HRV is an effective method to detect changes in the cardiac autonomic nervous system, even in asymptomatic individuals. Physical exercise can improve autonomic function in CD. Further clinical studies are needed to assess the anatomical-functional, metabolic, and immunological parameters associated with CD evaluated by HRV.


A doença de Chagas (DC) é um grave problema de saúde em países da América Latina, incluindo o Brasil, sendo caracterizada como infecção aguda e crônica. A DC pode afetar vários órgãos e tecidos celulares, em particular o coração, podendo levar à morte súbita. O objetivo deste trabalho foi revisar a literatura relacionada à função autonômica cardíaca avaliada pela Variabilidade da Frequência Cardíaca (VFC) na DC em indivíduos adultos. A busca foi realizada nas bases de dados eletrônicas PubMed, SciELO e Web of Science entre o período de janeiro de 2013 a outubro de 2020. Foram encontrados 19 artigos, dos quais 14 foram incluídos nesta revisão. A VFC foi demonstrada em condições basais, exercícios físicos e reabilitação cardíaca. Quantificar a progressão da DC avaliada pela VFC é um método eficaz para detectar alterações no sistema nervoso autônomo cardíaco, mesmo em indivíduos assintomáticos. O exercício físico pode melhorar a função autonômica na DC. Mais estudos clínicos são necessários para avaliar os parâmetros anátomo-funcionais, metabólicos e imunológicos associados à DC e avaliados pela VFC.


La enfermedad de Chagas (EC) es un grave problema de salud que enfrenta los países de América Latina y se caracteriza por ser una infección aguda y crónica. La EC puede afectar varios órganos y tejidos celulares, en particular el corazón, y puede provocar la muerte súbita. El objetivo de este estudio fue revisar la literatura que relaciona la función autónoma cardíaca evaluada por la Variabilidad de la Frecuencia Cardíaca (VFC) en la EC en individuos adultos. La búsqueda se realizó en las bases de datos electrónicas PubMed, SciELO y Web of Science desde enero de 2013 hasta octubre de 2020. Se encontraron 19 artículos, de los cuales 14 fueron incluidos en esta revisión. La VFC se ha demostrado en condiciones basales, ejercicio físico y rehabilitación cardíaca. La cuantificación de la progresión de la EC evaluada por la VFC es un método eficaz para detectar cambios en el sistema nervioso autónomo cardíaco, incluso en individuos asintomáticos. El ejercicio físico puede mejorar la función autónoma en la EC. Se necesitan más estudios clínicos para evaluar los parámetros anatómico-funcionales, metabólicos e inmunológicos asociados con la EC y evaluados por la VFC.

RESUMO Objetivo: analisar os sentidos produzidos pelo processo de adoecimento e como eles afetam as pessoas que vivem com tuberculose (TB) e o vírus da imunodeficiência humana (human immunodeficiency virus [HIV]). Método: trata-se de pesquisa qualitativa, com fundamentação teórico-metodológica da Análise de Discurso (AD) de matriz francesa, realizada em um centro de referência para infecções crônicas, no período de janeiro a julho de 2015. Utilizou-se um roteiro de entrevista semiestruturada para a coleta de dados primários. Para a coleta de dados secundários se recorreu ao livro de registro e acompanhamento de tratamento dos casos de TB; ao Sistema de Informação de Agravos de Notificação (SINAN); e ao sistema de controle de pacientes com TB. Resultados: foram entrevistadas 6 pessoas que realizam tratamento para TB e HIV e emergiram 3 blocos discursivos: "Tuberculose e HIV: sentidos circulantes"; "O vínculo paciente-profissional e utilização de incentivos na continuidade do tratamento"; e "Condicionantes que influenciam o processo de adoecimento". Considerações finais: a concepção de que esses agravos acometem apenas determinados indivíduos se mantém circulante na sociedade, favorecendo o preconceito, diagnóstico tardio e dificuldade de adesão ao tratamento. Assim, fazem-se necessárias políticas públicas e ações de promoção à saúde por parte dos gestores e profissionais da saúde para contribuir com a (des)construção de saberes e o cuidado integral e longitudinal tanto daqueles que estão em tratamento quanto de seus familiares.


RESUMEN Objetivo: analizar los sentidos producidos por el proceso de enfermedad y cómo estos afectan a las personas que viven con tuberculosis y VIH. Método: se trata de una investigación cualitativa, con fundamentación teórico-metodológica del Análisis del Discurso de matriz francesa, realizada en un centro de referencia para infecciones crónicas, en el período de enero a julio de 2015. Se utilizó un guion de entrevista semiestructurada para la reco-lección de datos primarios y, para la recolección de datos secundarios, se utilizó el libro de registro y acompa-ñamiento del tratamiento de los casos de tuberculosis; el sistema de información de agravios de notificación y el sistema de control de pacientes con tuberculosis. Resultados: fueron entrevistadas seis personas que realizan tratamiento para Tuberculosis y VIH y surgieron tres bloques discursivos: "Tuberculosis y VIH: sentidos circulan-tes"; "El vínculo paciente-profesional y utilización de incentivos en la continuidad del tratamiento"; y "Condicio-nantes que influyen en el proceso de enfermedad". Consideraciones finales: la idea de que esos agravios afectan solo a determinados individuos, se mantiene propagando en la sociedad, favoreciendo el prejuicio, diagnóstico tardío y la dificultad a la adhesión al tratamiento. Así, se hacen necesarias políticas públicas y acciones de pro-moción a la salud, por parte de los gestores y profesionales de la salud, que contribuyan para la (des)construcción de saberes y promuevan el cuidado integral y longitudinal a los que están en tratamiento, así como a sus familiares.


ABSTRACT Objective: analyze the meanings produced by the illness process and how they affect people living with tuberculosis (TB) and the human immunodeficiency virus (HIV). Method: this is qualitative research, with theoretical-methodological approach of French Discourse Analysis (DA), carried out in a referral center for chronic infections, within the period from January to July 2015. A semi-structured interview script was used to collect primary data. For the collection of secondary data, we used the record and follow-up book for TB treatment; the Notification Disease Information System (Sistema de Informação de Agravos de Notificação [SINAN]); and the TB patient control system. Results: 6 people undergoing TB and HIV treatment were interviewed and 3 discursive blocks emerged: "Tuberculosis and HIV: circulating meanings;" "The patient-practitioner bond and use of incentives in treatment continuity;" and "Conditions that influence the illness process." Final remarks: the theory that these health conditions affect only certain individuals remains circulating in society, favoring prejudice, late diagnosis, and difficulty in complying with treatment. Thus, public policies and health promotion actions taken by health managers and practitioners are needed to contribute to knowledge (de-)construction and comprehensive and longitudinal care both for those undergoing treatment and their family members.
